i remember a very graphic scene in almudena grandes' "the ages of lulu" where a man squats on the gloved fist of another man. where the glove has some sort of latex spikes, and the action is bloody intense, with all meanings intended

Sorry, but from how I "read" that scene in "Der Alte Affe Angst", he's just rubbing her pussy.

A scene that I always read the other way round even though there's no direct mentioning of fisting is in Fight Club. Remember how Edward Norton comes to the door of the room and Brad Pitt is wearing those gloves with Marla on the bed? Can't say why exactly, but I always thought "aha, gloves, so he was fisting her".

yes, we're talking of mainstream movies where one can't see but knows that some fisting is going on

I have three fisting-references, two from books and one film, all of which would have to be called main-stream.

First book - This one threw me slightly, since it's by no means an obscure author.
Stephen King, The dark tower, volume two - The drawing of the three. Chapter five, a few pages in you will find this passage.

I can't find the exact place, but it was an implied fisting, as a means to extract information. Tempus/Thales uses his armored fist in an undisclosed way that is apparently very painful and ultimately fatal to the subject, who is a eunuch and possibly gay or at least not uninterested in men. Tempus washes his armored fist in the sea after the 'extraction'.
