He apparently suffered extremely for weeks and even opiates could not help relieve the pain.

Read the J-Lube label: the warning about it being toxic to your innards is there for folks who use their product for extreme anal penetration. They know who they also sell their product to. It's not just used for animal husbandry, it's also used by special effects specialist when they make goo or slime (they used it in all the Aliens movie, it's obvious), extreme soap bubble competitions (yes, it's a thing) and people who take toys and hands up their butt. J-Lube is very aware that when they sell to shops in downtown New York or San Francisco, it's not used by cow boys (except maybe that guy from the Village People).

I have been injured once by a top who kept on insisting on going too far. I had to tell him to stop and then I stopped playing with him altogether. No bleeding right away but a few days later I passed blood but nothing extreme. A sexual confident of mine insisted on me going to the hospital but I saw the bleeding happened once and that was it, therefore I did not go.

Why I threw away all my old school toys: they were either made of a material that was too hard or had a shape designed by someone who is not a player or were made of toxic plastic.

Why I only use the expensive higher-end toys: they are made of a softer material and designed by people who are also aware of safety as well as what really works. 

I recommend against large butt plugs, because once inside, they tend to want to stay in and not come out. Some folks ended up in the ER because the base of the plug tore off after they tried to pull on it. I've almost found myself in that situation and managed to pull the largest butt plug I ever took (4.5" in diameter) after a lot of effort. I have since thrown it away. 

I also believe that you should not do drugs beyond a bit of cannabis and poppers. Cannabis can make you really high, though, so be careful. I have seen some videos of a guy continuously inhaling copious amount of weed smoked from a bong while being pegged with a huge and very long dildo. Errr... I don't see the enjoyment. But he did, apparently. Poppers, well, we talked about it. They can cause eye damage, lower your O2 sat, etc... etc... so use in small amount.

So here are my recommendations for extreme anal play (from someone who is not a medical doctor but has been playing for 35 years):

• Don't use hard drugs (although cannabis can almost be considered a hard drug nowadays)
• Don't play with a top who is high (if you take turns, then read the above: he should not be high on anything that clouds his judgement) or doesn't know what he's doing (I swear I am also a great top, if you're in the Phoenix area, PM me)
• Don't go too deep. If you take a fist or too, stay in the lower portion of the rectum. Seriously, the "alien trick" played by HKJ and DGG is just weird (my opinion) and freaks me out. If you still enjoy deep play, use a softer toy like the Slink. 
• Don't use hard or poorly designed toys or anything hard in that matter, beyond maybe the experimenting phase when you discover that butt play can be fun, you're 15 and live at your parents and all you have is a can of spray
• If you get injured, swallow your pride, go to the ER (read AMAs on Reddit, tons of nurses and ER doctors have had to deal with rectal foreign objects and they don't care, you're just a patient to them)
